ABSTRACT

Patients with spinal cord injury is associated with seriously affected gastrointestinal function and imbalance of intestinal flora, leading to increased inflammation of spinal cord nerves. With the proposal of the theory of gut microbiota-gut-brain axis in recent years, the regulatory role of gut microbiota in the central nervous system and gastrointestinal system has gradually attracted attention. Although a considerable number of studies have focused on the effects of intestinal flora characteristics on spinal cord nerve function repair in patients with spinal cord injury from different perspectives, there are numerous research models for treating spinal cord injury with intestinal flora as intervention targets and remains a lack of unified and effective clinical treatment methods. In this paper, the authors review the research progress in characteristics of intestinal microflora and intestinal microflora-targeted therapeutic methods in patients with spinal cord injury, hoping to provide a reference for the clinical treatment and basic research of spinal cord injury.

ABSTRACT

Objective:To observe the changes of protein expression of apoptosis signal pathway in prefrontal cortex of rats with post-stroke depression(PSD) after lateral ventricle injected of brain-derived neurotrophic factor precursor(proBDNF).Methods:Among 55 healthy adult female SD rats, 25 rats were randomly selected as PSD group, and the other 30 rats were randomly divided into normal group ( n=10), depression group ( n=10) and stroke group ( n=10). The middle cerebral artery occlusion(MCAO) model was established by thread occlusion in the stroke group, the chronic stress depression model in the depression group was established by the combination of chronic unpredictable mild stress(CUMS) and the solitary feeding method.And the rats in the PSD group were established MCAO model first, then they were received CUMS stress and solitary rearing one week later so as to establish PSD model.Two weeks after the establishment of the model, 15 rats in PSD group were randomly divided into proBDNF group, rats in tPA group and NS control group.One week after buried tube of lateral ventricle, rats in tPA and proBDNF were injected into the lateral ventricle for one week.The protein expressions of c-Jun N-terminal kinase(JNK), p-JNK, p53, p-p53 and Bax in prefrontal cortex of rats in each group were detected by Western blot at the 4th and 8th week after modeling.SPSS 17.0 software was used for data analysis, one-way ANOVA was used for comparison between groups, and SNK- q was used for pairwise comparison. Results:The expressions of p-p53, p53, p-JNK, JNK and Bax in prefrontal cortex of normal group, depression group, stroke group and PSD group were significantly different at the end of 4th and 8th week after MCAO modeling ( F=3.426-90.355, all P<0.05). Post-hoc analysis showed that, compared with the normal group, the expressions of p-JNK (0.378±0.042) and Bax (0.478±0.054) in the prefrontal cortex of PSD rats increased significantly at the end of the 4th week(both P<0.05), and the expressions of p-JNK(0.411±0.056), p-p53 (0.286±0.083) and Bax (0.471±0.008) in the prefrontal cortex of PSD group increased significantly at the end of the 8th week(all P<0.05). After lateral ventricle injection of proBDNF, there were significant differences in the expression of p-p53, p53, p-JNK, JNK and Bax among proBDNF group, tPA group and NS group ( F=16.915-287.039, all P<0.01). Post-hoc analysis showed that, compared with NS group, the expressions of p-JNK (0.35±0.01)and p-p53 (0.31±0.01)in prefrontal cortex of proBDNF group increased significantly(both P<0.05). After lateral ventricle injection of proBDNF, there were significant differences in body weight, sucrose preference rate, horizontal movement distance among proBDNF group, tPA group and NS group ( F=18.741-76.305, all P<0.01), and compared with tPA group and NS group, behavioral indexes of proBDNF group (body weight (224.36±3.23) g, sucrose preference rate (69.83±1.72)%, horizontal movement distance (57.93±2.09) blocks, vertical movement distance (19.79±1.81)) decreased significantly(all P<0.05). Conclusion:The proBDNF promotes the activation of apoptosis signal pathway in the rats with PSD.

Objective@#To explore the effect of quality control circle(QCC)on reducing the incidence of moderate or severe malnutrition in gastric cancer patients under chemotherapy.@*Methods@#Before the activity (February to March 2016), 146 gastric cancer patients under chemotherapy were selected as the control group, and regular nutrition nursing (dietary guidance and regular nutrition-related health lectures) was given. After the activity (February to March 2017), 139 gastric cancer patients under chemotherapy were selected as QCC group, and the standard intervention of quality control circle was implemented. The nutritional status of patients before and after improvement was compared.@*Results@#After finding the cause and formulating and implementing the measures, the 24-hour consultation rate, the incomplete remission rate of nausea/vomiting, the incidence of cancer anorexia, and the rate of missed visits in QCC group were significantly different with those in control group (P<0.05). The incidence of moderate or severe malnutrition was 16.55% in QCC group and 65.07% in control group, and the difference was statistically significant (P<0.01).@*Conclusions@#The quality control circle activity can effectively reduce the incidence of moderate or severe malnutrition and improve the quality of life in gastric cancer patients under chemotherapy. It can also improve the self-discipline and initiative of medical staff and improve the quality of medical care.

ABSTRACT

Objective To evaluate the intervention effects of peer support education mode for type 2 diabetes control in rural residents.Methods A random cluster sampling method has been used,including 300 rural residents aged above 18 years old from three villages (184 in control group,116 in intervention group),in order to proceed the physical check-up and health education programs.Unchanged rate,transfer rate of patients,rate of impaired glucose tolerance,turn normal rate and other biochemical indicators of patients and people with impaired glucose tolerance from control group and intervention group were analyzed,to evaluate the intervention effects of peer support education mode.Results The glycemic control rate of intervention group for patients and people with impaired glucose tolerance (72.2% and 71.4%) were higher than control group (43.6% and 26.7%),but the unchanged rate of intervention group (13.9% and 0.0%) were lower than control group (42.3% and 73.3%).Patients with diabetes or glucose intolerance in the education group improved significantly in waist-to-hip ratio,uric acid,total cholesterol and HDL-C.Glycemic hemoglobin level also improved significantly in diabetes patients of the education group.Conclusion Peer support for education intervention seemed beneficial for diabetic control.The combination of education and effect evaluation was important in the evaluation of diabetes prevention and control.Peer support education also benefited the blood glucose control in general population.

ABSTRACT

Objective To evaluate the value of 3.0T magnetic resonance multi-b value diffusion-weighted imaging (DWI) in evaluating the efficacy of chemotherapy for patients with central lung squamous cell carcinoma and atelectasis. Methods Twenty patients with lung squamous cell carcinoma were examined by magnetic resonance imaging (MRI) (including T1WI, T2WI and multi-b value DWI) before chemotherapy, 2 cycles of chemotherapy and 4 cycles of chemotherapy. The images, the tumor volume and changes of apparent diffusion coefficient (ADC) were analyzed. Results In the patients with central lung cancer and atelectasis, the tumor and atelectasis could be distinguished on MRI examination before radiotherapy. It was more easily identified on T2WI images after radiotherapy. In the 20 patients, the ADC values in the effective group (partial remission or complete remission) and the invalid group were increased, but the differences of ADC values in the effective group before chemotherapy, 2 cycles and 4 cycles of chemotherapy were statistically significant [b=800 s/mm2:(1.09 ± 0.52) × 10-6 mm2/s, (1.22 ± 0.59) × 10-6 mm2/s, (1.24 ± 0.52) × 10-6 mm2/s, F = 31.19, P < 0.001]. There was no significant difference in ADC values between before and after chemotherapy (b = 800 s/mm2: (1.10 ± 0.49) × 10-6 mm2/s, (1.16 ± 0.60) × 10-6 mm2/s, (1.20 ± 0.72) × 10-6 mm2/s, F=2.86, P=0.089]. When b=800 s/mm2, the ADC curve slope in the effective group was more stable, better linearity. Conclusions The MRI technique can accurately distinguish the tumor from atelectasis before and after chemotherapy. The change of ADC value after chemotherapy is earlier than that of morphological change. The change rate of b value can better evaluate the curative effect of chemotherapy.

ABSTRACT

Objective To evaluate the effect of sedation with regional anesthesia with dexmedeto-midine on postoperative cognitive function in elderly patients with diabetes mellitus. Methods A total of 180 elderly patients of either sex with diabetes foot, aged 65-80 yr, weighing 45-90 kg, of American So-ciety of Anesthesiologists physical status ⅡorⅢ, with preoperative Mini-Mental State Examination score>24, undergoing elective débridement, were divided into 2 groups(n=90 each)using a random number table: sedation with dexmedetomidine group(group D)and routine sedation control group(group C). Lumbar plexus block and sciatic nerve block were performed, after the reliable efficacy was achieved, dexmedetomidine was intravenously infused in a dose of 0.5 μg∕kg for 10 min followed by an infusion of 0.5 μg·kg-1·h-1until the end of surgery. Midazolam 0.02-0.04 mg∕kg was intravenously injected and mid-azolam 1 mg was injected intermittently during operation in group C. Ramsay sedation scores were main-tained between 2 and 4. At 30 min before surgery(T1), 1 h after the beginning of surgery(T2), at the end of surgery(T3), 6 h after the end of surgery(T4), and 24 h after the end of surgery(T5), venous blood samples were collected for determination of the level of blood glucose and plasma cortisol(Cor)con-centrations. Mini-Mental State Examination scores were assessed at 1 day before surgery and 1 and 3 days after surgery, and the occurrence of postoperative cognitive dysfunction was recorded in a short time period after operation. Results Compared with the baseline at T1, the level of blood glucose at T2-5and plasma Cor concentrations at T3,4were significantly increased in group C, and plasma Cor concentrations were sig-nificantly increased at T3,4(P<0.05), and no significant change was found in the level of blood glucose at T2-5in group D(P>0.05). Compared with group C, the level of blood glucose at T3-5and plasma Cor con-centrations at T3,4were significantly decreased, Mini-Mental State Examination scores were increased at 1 and 3 days after operation, and the incidence of postoperative cognitive dysfunction in a short time period after operation was decreased in group D(P<0.05). Conclusion Sedation with regional anesthesia with dexmedetomidine can improve postoperative cognitive function in elderly patients with diabetes mellitus.

ABSTRACT

OBJECTIVE:To investigate the efficacy and safety of Xiaojin pill combined with docetaxel,epirubicin and cyclo-phosphamide in the treatment of breast cancer in stage Ⅲ and its effect on immune functions. METHODS:102 patients with breast cancer in stage Ⅲ were randomly divided into control group (51 cases) and observation group(51 cases). All patients were given symptomatic treatments as hydration,alkalization and antiemetic treatments in the duration of chemotherapy,and were given conven-tional treatments as 0.75 mg Dexamethasome oral tablets in 12 and 6 hours before the chemotherapy lespectively. Control group was given 75 mg/m2 Docetaxel injection,d1,intravenous infusion+60 mg/m2 Epirubicin hyclrochloride for injection,d1,intravenous in-fusion+600 mg/m2 Cyclophosphamide for injection,d1,intravenous infusion;observation group was additionally given 6 g Xiaojin pill,orally,twice a day. 21 d was regarded as a treatment course. Short-term clinical efficacy,immunoglobulin,T lymphocyte sub-sets(CD4+/CD3+,CD8+/CD3+,CD4+/CD8+)and the incidence of adverse reactions before and after chemotherapy in 2 groups after 2 courses were observed. RESULTS:There were no significant differences in the short-term effective rate and incidence of adverse reactions between 2 groups (P>0.05). Before chemotherapy,there were no significant differences in the immunoglobulin and T lymphocyte subsets between 2 groups(P>0.05). After chemotherapy,immunoglobulin levels in 2 groups were significantly lower than before,the differences were statistically significant (P0.05);CD4+/CD3+,CD8+/CD3+,CD4+/CD8+ in control group and CD8+/CD3+ in observation group were significantly lower than before,and CD4+/CD3+ and CD4+/CD8+ in observation group were higher than control group,the differences were statistically significant (P0.05). CONCLUSIONS:Based on the conventional treatment,Xiaojin pill combined with docetaxel,epirubicin and cyclophosphamide shows significant effi-cacy in the treatment of breast cancer in stageⅢ,and can improve immune functions,with good safety.

Objective One hundred and sixteen cases of pediatric patients with severe heart fail-ure,pneumonia combined application of phentolamine dopamine synthesis with conventional medical treat-ment were compared the clinical efficacy,analyze and judge the feasibility of both combination therapy clini-cal significance and its value.Methods Collected from July 2013 to April 2015 period to the author hospi-tal and diagnosed with pneumonia in children with heart failure in 116 patients.Which were randomly divid-ed into two groups,group A +phentolamine medical treatment with dopamine and B group routine clinical medicine comprehensive treatment.Clinical observation by a number of factors to analyze comparative A,B two groups after the treatment efficacy.Results A significant effect of the treatment group and the number of effective treatments were more than group B and A group of overall response rate (91.38%)than in group B (60.34%);A group invalid proportion treated patients (8.62%)was significantly less in group B (39.66%);A group of children mortality (5.17;Change a group on heart rate and blood pressure had a better than group B;and the difference was greater (P 0.05)Conclusions A group of overall efficiency significantly higher in group B and body recover faster in children,low mortality,children with severe pneumonia prompted phentolamine and dopamine treatment with heart failure better.

ABSTRACT

Objective To evaluate the value of CT and MRIin diagnosis of littoral cell angioma(LCA) of spleen.Methods CT and MRI findings the data of 6 patients with LCA were rospectively analyzed,and all patients were proved LCA of spleen by pathology.Then enhanced CT (n =4 cases) and MRI (n =2 cases) were performed.The imaging performance of LCA was observed,and its imaging features was summarized.Results None of patients showed obvious signs and symptoms of hypersplenism,4 cases were with splenomegaly.5 patients had a single lesion,only one had few lesions.There were not any calcification or envelopement of all the lesions.On CT,all lesions demonstrated distinct borders.The enhanced scan for low-density nodules demonstrated slow progressive enhancement.On MRI,all the LAC had well circumscribed borders,and demonstrated T1 · hypointense and T2-equal,hyperintense signals,and progressive enhancement on the post contrast images.Conclusion CT and MRI imaging of littoral cell angioma of spleen have special imaging characteristics,those particular findings is helpful in the diagnosis.

ABSTRACT

ObjectiveTo compare the clinical value of contrast-enhanced ultrasonography (CEUS) and conventional ultrasonography in diagnosing polypoid lesion of gallbladder (PLG).MethodsThe postoperative pathology and clinical manifestation of 49 PLG patients were retrospectively analyzed in the First Afifliated Hospital of Harbin Medical University. Of them, 7 cases were maligant lesions, 4 cases were adenomas, 24 cases were polyps of gallbladder and 14 cases were adenomyomatosis of gallbladder. With postoperative pathology as golden standard, the sensitivity, speciifcity and PLG speciifcity were calculatedfor conventional ultrasonography, CEUS and conventional ultrasonography plus CEUS.ResultsThe sensitivity, specificity and PLG specificity were 33.3%, 74.3% and 68.3% for conventional ultrasonography, and 66.7%, 85.7% and 82.9% for CEUS respectively. The conventional ultrasonography plus CEUS were 83.3%, 88.6% and 87.8%.ConclusionCEUS could signiifcantly improve the diagnostic accuracy.

ABSTRACT

Objective To explore effect of different withdrawal time of dexmedetomidine (DEX) on the quality of general anesthesia recovery.Methods Eighty patients of ASA Ⅰ or Ⅱ undergoing lymph surgery were randomly assigned to four groups (n =20).Groups D1,D2 and D3 received DEX 0.5 μg/kg as bolus before induction,continued with 0.5 μg/(kg · h) by infusion until one hour,30 min before the end of operation and the end of operation,respectively.Group C received equal volume of normal saline.Mean artery pressure (MAP),heart rate (HR) and pulse oxygen saturation (SpO2) were observed during and after operation.Spontaneous respiration recovery time,eyes open time,extubation time,orientation recovery time,observer's assessment of alertness/sedation score (OAA/S),restlessness score (RS),and visual analogue scales (VAS) were observed after operation.Results For MAP and HR in Groups D2 and D3,there were no statistically significant difference during extubation compared to those at preoperation,but at the same time point,they were lower than those in group C (P < 0.05).For groups C and D1,MAP and HR at extubation and 5 min after extubation were higher than those at preoperation (P <0.05).Spontaneous respiration recovery time,eyes open time,extubation time,orientation recovery time in group D3 were significantly longer than those in Group C (P < 0.05),while there were no statistically significantly difference between groups D1,D2,and group C.OAA/S in group D3 was significantly lower than that in group C (P < 0.05) at extubation.Incidence of restlessness,VAS,and cases given analgesic 2 hours after operation in groups D2 and D3 were significantly lower than those in groups C and D1.Compared to those in group C,dosages of propofol and remifentanil in groups D2 and D3 were significantly lower.Conclusions Dexmedetomidine administered of 0.5 μg/kg before induction,continued with infusion of 0.5 μg/(kg · h) until 30 min before the end of operation,may improve emergence,without influencing the awakening time of patients,and prolong the duration time of analgesia which comfort the patients.

ABSTRACT

Objective Make a scientific approach to the validity of 3D-VIBE MRI on breast tumor's evaluation.Methods 141 breast processes were examined by 3D Dynamic scanning technique with high speed and resolution, Compared with postop pathological appearance, the diagnosis of breast tumor, the ductal carcinoma in situ, their circumscriptions, and axillary lymph node metastasis were evaluated. Results The coincidence of tumor existence diagnosis was 97.2 %(137/141). The accuracy, sensitivity and specificity of progression in duct were 69.4 %(93/134), 75 %(100/134), 57.1%(77/134), respectively.The accuracy,sensitivity and specificity of axillary lymph node metastasis were 92.5 %(124/134), 53.8 %(72/134), and 96.7 %(129/134) respectively. Conclusion The reconstruction images through 3D Dynamic scanning technique with high speed and resolution could discover minute breast tumor and the extent of axillary lymph node metastasis around breast,and the extent of diseases in duct can also be effectively evalnated.

ABSTRACT

The levels of macrophage migration inhibitory factor (MIF) in peripheral blood mononuclear cell (PBMC) and serum in patients with Hashimoto's thyroiditis (HT) were detected by real-time PCR and ELISA,respectively.The results revealed that the expression of MIF mRNA in PBMC ( Z =-4.276,P<0.01 ) and protein level in serum ( Z=-5.358,P<0.01 ) were increased in HT patients,and positively correlated with thyroid specific autoantibodies and TSH levels.

ABSTRACT

Objective To assess the value of imaging examination technologies such as 2-D ultrasound(2-DUS),helical computed tomography (CT),contrast-enhanced ultrasonography (CEUS) and contrast-enhanced CT (CECT) in the positioning and qualitative diagnosis of pancreatic diseases.Methods 2-DUS,helical CT findings were comparatively reviewed to analyze in 44 cases(45 lesions) with pancreatic space-occupying lesions verified by operation and pathology,and investigated the enhanced mode and the perfusion features of CEUS and CECT in different phases.Results In 45 lesions,there were 29 malignant lesions and 16 benign lesions.CEUS and CECT had greater diagnostic value compared to 2-DUS and helical CT.The sensitivity,specificity and accuracy of 2-DUS for the malignant lesions were 72.41 %,62.50% and 66.67 %,while the helical CT were 68.97%,75.00 % and 68.89 %.The sensitivity,specificity and accuracy of CEUS for the malignant lesions were 93.10%,87.50% and 91.11%,while the CECT were 86.21%,93.75% and 86.67%.There was no statistical difference between CEUS and CECT in detecting pancreatic space-occupying lesions.Conclusions CEUS and CECT can provide more accurate diagnosis on for pancreatic space-occupying lesions than 2-DUS and helical CT.Following CECT,CEUS is a new technology for diagnosis on pancreatic space-occupying lesions.

ObjectiveTo investigate the changes in the expression of T-cell death-associated gene 8(TD- AG8) in spinal cord in rats with bone cancer pain.MethodsTwo hundred and twenty-four female rats weighting 150-180 g were randomly divided into 3 groups:normal control group(group Ⅰ,n = 64),normal saline group (group Ⅱ,n = 64),bone cancer pain group(group Ⅲ],n = 96).Bone cancer pain was induced by inoculating Walker256 mammary gland carcinoma cells into the tibia medullary cavity.Mechanical withdrawl threshold(MWT)was measured at 1 d before(baseline)and 1,3,6,9,12,15 and 18 d after inoculation.Sixteen rats were sacrificed at 1 day before(baseline)and 6,9,12,15 and 18 d after inoculation in group Ⅲ and 18 d after inoculation in groups Ⅰ and Ⅱ.The L4-6 spinal cord were removed,and the number of TDAG8 positive cell was counted,and the expression of TDAG8 mRNA was measured by RT-PCR.ResultsCompared with baseline value and group Ⅰ,MWT was decreased,and the number of TDAG8 positive cells and the expression of TDAG8 mRNA in spinal cord were increased at 6-18 d after inoculation in group Ⅲ ( P < 0.01 ).ConclusionThe expression of TDAG8 in spinal cord is up-regulated in rats with bone cancer pain,which may be involved in the mechanism of the development of bone cancer pain.

ABSTRACT

Objective To evaluate the role of gliocytes in the spinal cord in the development of inflammatory pain (IP) in rats. Methods Adult male SD rats weighing 180-220 g were used in this experiment. A catheter was implanted in the subarachnoid space according to the method described by Yang. Animals with abnormal motor function of the hindlimb after intrathecal (IT) catheter implantation were excluded. IP was induced by subcutaneous (sc) injection of complete Freund's adjuvant (CFA) 50 μl at the lateral side of the ankle joint of the right hindpaw. Sixty-five rats were randomly divided into 5 groups ( n = 13 each): group I IP control normal saline (NS) 50μl was injected sc instead of CFA; group II IP; group IE PC (IT) + IP control fluorinated citric acid (FC, a gliocyte metabolism inhibitor) 1 nmol/10μl was injected IT at 15 min before NS 50 μl sc injection; group IV NS (IT) + IP and group V FC (IT) + IP. The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured 2 d before induction of IP (T_0, baseline) .before and at 2, 4, 6, 8, 10, 12, 24 and 26 h (T_(1-9)) after sc NS or CFA injection. Five enimals in each group were killed at T_5 (8 h after sc NS/CFA injection) and the lumbar segment (L_(4,5)) was removed for determination of glial fibrillary acidic protein ( CFAP) and OX-42 expression by immuno-histochemistry. Results In group Ⅱ and Ⅳ sc CFA significantly decreased MWT and TWL. Mechanical and thermal hyperalgegia induced by sc CFA was significantly suppressed by intrathecal FC in group V . IP significantly increased GFAP and OX-42 expression in the spinal cord. Intrathecal FC significantly attenuated IP-induced up-regulation of GFAP and OX-42 expression in the spinal cord. Conclusion The activation of gliocytes in the spinal cord is involved in the development of CFA-induced hyperalgesia in rats.

ABSTRACT

Objective To provide the evidence for the clinical prevention and early treatment for the complication by testing microcirculation of the middle fingertip in renal failure patients with hemodialysis.Methods High-frequency ultrasounds combined with E-flow technique were applied to detect the Doppler velocitg index of middle fingertip's nail bed arteriole and ventral arteriole in 35 renal failure patients with hemodialysis.Peak systolic velocity(PSV),end-diastolic velocity(EDV)and vascular resistance index(RI)were separately measured,and comparison were made with healthy adults and renal failure patients without hemodialysis(sex and age matched).Results Compared with normal control,PSV and EDV of middle fingertip's nail bed arteriole and ventral arteriole were degraded statistically(P<0.05),while RI was increased statistically(P<0.05)in renal failure patients with and without hemodialysis.Compared with renal failure patients without hemodialysis,PSV and EDV were degraded(P<0.05)and RI was increased(P<0.05)in renal failure patients with hemodialysis.Conclusions High-frequency ultrasound combined with E-flow imaging technique can evaluate the microvascular peripheral resistance in renal failure patients with hemodialysis.

ABSTRACT

Objective To investigate the value and the enhanced mode of time-intensity curve(TIC)combined with gamma curve of contrast-enhanced ultrasound in diagnosis of hepatocellular carcinoma.Methods The ultrasonic contrast images of 34 patients with hepatocellular carcinoma by the gamma curve were obtained by the time before,increased branch slope parameter, curve sharpness, gamma modulus, peak time,peak intensity, the average pre-peak intensity, increased branch slope and pre-peak area.Results Compared with the peripheral hepatic parenchyma, the curve of the tumor tissue ascended as well as declined more rapidly, peaked earlier with a more intensive peak value.There were significant differences between the two groups in all the data mentioned-above (P<0.05 ).Conclusions TIC and gamma fit quantitative analysis can reflect the different perfusion characteristics between hepatocellular carcinoma and normal hepatic parenchyma objectively and accurately,so as to provide the quantitative criterion and basis for the diagnosis and hemodynamic study of the hepatocellular carcinoma.
